What is a long-term condition

A long-term condition has a significant impact on people’s lives. It can be anything from asthma, cancer, weight problems, diabetes, COPD respiratory, depression, cardiovascular disease (including stroke and heart failure), mental illness (depression and anxiety), chronic pain, fibromyalgia, chronic kidney disease – anything that stops you from living a well, healthy and active life.

About Us

The Kia Ora Programme / Stanford Chronic Disease Self-Management Programme is FREE to attend and is designed for individuals living with a long-term condition, as well as those supporting whānau or friends with ongoing health needs. It is an internationally recognised course developed by Stanford University, equipping participants with practical tools and skills to better manage long-term health conditions over the duration of the programme.
